Mountain Community Health Partnership (MCHP) is seeking a full time Behavioral Health Provider. The Behavioral Health Provider (BHP) is responsible for providing integrated behavioral health services to MCHP patients; especially under-served, uninsured, under-insured, high-risk and vulnerable patient populations residing in the service area of MCHP regardless of their ability to pay for services. These duties include serving as a direct provider of behavioral health services providing quality, compassionate, and culturally sensitive care for patients of MCHP according to evidence-based best practices for integrated behavioral health care. The BHP supports the care teams in coordination of patient care by provider or case manager referral and shared visits. Responsibilities include, but are not limited too, providing integrated behavioral healthcare services to clinic patients. BHP will be available to see patients at the time of their primary care visits as much as possible. BHP will use motivational interviewing to assess patient motivation and provide brief intervention at time of medical visit, schedule follow up with BHP and/or follow up with another agency; provide "curbside" consults to medical providers as needed; review patient filed/record with attention to details of behavioral health history and health behavior changes; refer patients to behavioral health specialist for consultation when necessary; serve on community committees and boards as schedule and scope of practice permits; participate in scheduled team meetings; presents and participates in training for care team to support patients/families/caregivers in self-management, self-efficacy and behavior; work with Medical Director, Healthcare Providers, Quality Coordinator; Front Office and Clinical Staff to improve management of patients.

Must have completed a Masters of Social Work (other educational backgrounds may be considered depending on clinical needs). Minimum of two years supervised employment, preferably with experience in primary care. Current and valid NC Licensed Clinical Social Worker.

The Behavioral Health Provider may be asked to float at any/all MCHP locations based on clinic need.

Pay: $61,700.00 - $74,000.00 per year
