Title: Case Manager & Navigator

Department: North Modesto or Merced

Salary Range: $16.00 - $20.87

Base rate is dependent on related years of experience

Position Summary: We are a nonprofit organization with a mission to strengthen families and communities by transforming lives! We are currently recruiting for Case Manager and Navigator who are ready to make a difference in the lives of others.

Qualifications:

1. Bachelor's degree in counseling, social work or related field preferred. Will accept equivalent experience.

2. Knowledge of community resources.

3. Cultural sensitivity awareness and knowledge.

4. Bilingual in Spanish preferred

5. Strong Communication Skills

6. Clean and neat appearance and pleasant demeanor

7. Must be 21 years old or over.

8. A valid California driver’s license with less than two minor moving violations is required of staff who have driving duties to meet the requirements of their position and be approved by the agency’s insurance company.

Essential Job Functions:

10. Provide transportation in approved vehicles in critical situations.

11. Sound personal qualifications (i.e., maturity, stability, high moral standards, and sound mental health and physical ability to perform job duties).

Responsibilities:

1. Provide advocacy and support in schools, preschools, community (neighborhoods), homes and agency as needed.

2. Participate in required training throughout the year.

3. Attend designated meetings unless excused by the Program Supervisor.

4. Provide outreach services to parents to include identifying areas that need to be strengthened, setting goals, and helping to achieve targeted goals with the families.

5. Assist families in providing resources and support to families as needed.

6. Facilitate, assist in children’s group as assigned.

7. Plan and develop activities.

8. Prepare childcare setting: clean toys, organize materials, set up room prior to each group beginning, clean room following each group and other duties in this area as needed.

9. Observe confidentiality of information related to other families.

10. Interact productively within a multi-disciplinary team to provide services to an identified group of families.

11. Maintain record keeping and tracking systems of assigned families.

12. Respond appropriately to familial situations.

13. Relate directly with community organizations.

14. Work productively as a team member.

15. Attend community events as directed.

Other:

1. Attend all required meetings.

2. Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.

3. Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.

4. Other duties as assigned.
