ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Ensure that consumers are living in safe, clean and decent environments.
- Document minimum of 2 progress notes per week.
- Ensures consumers have adequate ADL skills by demonstrating, training and coaching.
- Assist consumer with assessing needs and strengths to set goals and objectives.
- Writes the admission notes in the uniform case record.
- Documentation of progress notes as they pertain to consumers’ individualized service plans, and ensures restorative services are clearly illustrated in progress notes.
- Ensures that consumers are linked to all necessary ancillary services including psychiatric, medical and dental treatment, vocational and educational resources, and legal and financial entitlements.
- Provide crisis intervention in acute episodes of emotional and physical distress in order to contain consumer’s regression and to ensure safety of consumers.
- Completes follow-ups 30, 60 and 90 days documentation of discharged consumers.
- Represents the agency at conferences and meetings of referral sources, treatment providers and professional and community groups.
- Writes the individual service plans, service plan reviews in a timely manner for all consumers of the assigned caseload.
- Ensures that consumers have appropriate seasonal clothing.
- Monitor and evaluate whether or not consumers’ needs have been met by the ancillary services and make recommendations to supervisor.
- Assist consumers with entitlements, budgeting and managing utilities.
- Ensure that furnishings, livings and household items meet agency standards.
- Escorts consumers to housing, benefits, and psychiatric/medical appointments when required.
QUALIFICATIONS

Candidates must have an Associate’s degree and at least 2 years of Mental Health experience, particularly in the area of case management; BSW and CASAC are desirable, good interpersonal, writing skills and computer skills – experience with AWARDS preferred and bi-lingual in English and Spanish a plus.

· The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This position will require extensive walking when visiting consumers in various apartments throughout the city.
