Details about this exciting opportunity:
- Temporary (1 year) Full-Time Case Manager for our Young People Achieve program located in Ossining, Sleepy Hollow and White Plains schools.
- Compensation: $45k-$55k, depending on level of education.

Responsibilities include (but are not limited to):
- Provide direct service in various therapeutic modalities, completes intakes/assessments, maintains ongoing cases, attend Child Study Team/Intake meetings as applicable and provides crisis intervention and/or home visits as needed.
- Facilitate support group for pregnant and parenting teens remote and in person.
- Provide Health Class presentations in districts throughout Westchester.
- Responsible for all recording and statistical procedures required by the agency including, but not limited to, case records, insurance forms and related data, monthly, quarterly, and annual statistical sheets, schedules, surveys, etc.
- Maintain necessary patient related contacts with other agencies and resources (courts, schools, etc.) including written material, telephone contact and personal visits as necessary.
- Represent agency at community meetings.
If you have the following:
Qualifications:
- A Bachelor’s Degree in Social Work is required, a Master’s Degree in Social Work is preferred.
- Prior care management experience.
- Ability to make home visits either by using own vehicle or using public transportation throughout Westchester County.
And Attributes:
- Ability to work with teens and adolescents.
- Good knowledge of community resources and how to use them.
