Summary: Provides a variety of case management, skills training, support services as required for persons with Developmental Disabilities, Severe and Persistent Mental Illness, and Chemical Dependency as needed for the level of the position.
QUALIFICATIONS: The requirements listed below are representative of the knowledge, skills and/or abilities required to perform the essential functions of this job.
Knowledge, Skills and Abilities :
- Must be able to communicate effectively in both oral and written forms.
- Customer service and detail oriented.
- Comprehensive knowledge of evidence-based practices, including: group counseling skills, one-on-one counseling skills, treatment planning, assessments, and referrals.
- Experience with electronic medical records and telemedicine highly desirable
- Knowledge of cultural and family issues that can contribute to Lifeways’ development of clinically relevant and effective care.
- Bilingual/multilingual a plus
Minimum Requirements:
- Education and/or Experience: This is an entry level professional position with a BA degree in social work, behavioral health or related field. plus, two-years’ experience or an equivalent combination of education and experience with individuals with severe or persistent mental illness, or alcohol and drug dependency.
