Job description
Citizens Memorial Hospital (CMH) is recruiting a specialist to improve the care and health outcomes of patients.
Description of department: The Population Health team supports the clinics by working with payers to improve patient health.
A typical day will include: Contacting patients to schedule wellness visits, assisting clinics to get patients in for chronic condition follow ups, entering as well as responding to EMR tasks appropriately and timely. The department works on reporting that information back to the patient’s insurance payer. The goal of population health is to improve patient outcomes and increase health capital. Other goals include preventing disease and closing care gaps. The care gaps team provides excellent customer service and behaves in a manner consistent with the mission of CMH. Performs other duties as requested.
The position is: Full-Time Days Monday - Friday
