Clinical Manager of Intake and Referral (Registered Nurse, RN)

Remote Hybrid Opportunity

Full Time – 40hrs

The Intake and Referral department is the core of home health & hospice operations in any agency. We are seeking a clinical manager (RN) to lead and support our Intake and Referral department's day-to-day clinical and administrative activities and ensure implementation of/adherence to clinical policies/procedures. With our focus on moving patients appropriately through our integrated, academic health system to receive care in the most appropriate setting, the Clinical Manager of Intake and Referral is responsible for planning, organizing, leading, supervising and educating the Intake and Referral team in the provision of effective and efficient intake services.

This position will support our priorities of patient access to services (by moving patients effectively and efficiently through the intake process), patient safety (through review of clinical documentation and verification with referring providers), and financial health (by capturing patients appropriate for admission to home health & hospice services).

Furthermore, the Clinical Manager position is responsible for supervising and supporting the 6 registered nurses who are responsible for reviewing clinical orders and documentation in order to assure appropriateness of referrals for home health and hospice services; the team is also comprised of 5 non-clinical staff who provide crucial support for day-to-day intake and referral activities. The Clinical Manager will be responsible for ensuring Intake and Referral team members comply with, and participate in, mandated orientation, education, and professional standards and for compliance with timely submission of all clinical documentation in accordance with organizational policies and procedures.

MINIMUM QUALIFICATION STANDARDS: I. Education:
Graduate from an accredited school of nursing.

II. Experience:
  • Preferred, two or more years of directly related home health or hospice nursing experience or experience as a clinical nurse educator.
  • Or, four to six years of relevant clinical experience or a combination of education and experience from which comparable knowledge and skills are acquired preferred.
  • Previous experience managing multiple employees responsible for complex clinical activities.
III. License Requirements:
Current RN licensure or compact licensure recognized by the State of Vermont is required.
