Position Overview:

Provides evaluation, treatment and follow-up to patients within a clinical nursing program. Serves as a primary point of contact for all things related to the care and education of the patients and ensures efficient and effective processes that support safe patient care throughout continuum of care. Participates in the development and implementation of clinical programs and procedures and participates in performance improvement programs. Coordinates and supervises nursing standards of practice for patients and participates in data reporting. May also be responsible for performing specific procedures and/or orient other staff to the department.

EDUCATION:
Graduate of an accredited school of nursing Required.

BSN Preferred

CERTIFICATION & LICENSURE:
RN-Registered Nurse of California Required

BLS-Basic Life Support Certification Required


TYPICAL EXPERIENCE:

2 years recent relevant experience. (ICU, ED, Telemetry)

Patient navigation, care coordination, Cardiology background, some knowledge of imaging procedures and testing.

SKILLS AND KNOWLEDGE:

Demonstrated knowledge and competence with regard to nursing theories, concepts and practices; medical terminology; anatomy; and physiology.

Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.

Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
Possess strong written and verbal communication skill to collaborate with medical staff for case conferences and treatment planning.
Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record (EPIC preferred). Excel, Power Point

Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.

Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with competing interruptions.


Work independently to troubleshoot matters, communicate obstacles and move caseloads forward despite reliance on other offices for scheduling.


Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.

Ensure the privacy of each patient’s protected health information (PHI).

Build and maintain collaborative relationships with peers and other healthcare providers to achieve departmental and corporate objectives.

Possess ability to accomplish tasks within established workflows with other provider office staff and healthcare workers.

Participate in data collection and entry into databases.

Act as a resource and point of contact for Structural heart program.

PAY RANGE: $76.68 - $101.21 / Hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
