JOB DESCRIPTION

Summary of Responsibilities: The following position is for a person who will, and can assist older persons who are functionally, physically and/or socially impaired and need 24 hour oversight. This person must be willing and able to fulfill their responsibilities in a manner that is consistent with the philosophy of assisted living and United Faith Housing. United Faith Housing is an outreach ministry of thirteen local religious organizations.

Essential Duties and Responsibilities:

  • Activities of Daily Living (ADL’s) – Provides assistance with the following:
  • Dressing and undressing (clothing must be clean and appropriate for the season)
  • Bathing
  • Personal hygiene (shaving, dental hygiene, nail care, hair care & foot care).
  • Mobility devices (cane, walker, wheelchair & crutches) including the monitoring of safe transfer and ambulation techniques. Also, assist with transfers as needed.
  • Care of ADL device such as eyeglasses, contact lenses and hearing aids.
  • Health Maintenance and Monitoring Tasks:
  • Observes and reports changes in residents’ physical condition and cognitive/emotional status to Q.M.A. or D.O.N., as needed.
  • Conducts apartment checks and resident rounds.
  • Monitors for environmental safety hazards.
  • Records incidents, errors and accidents.
  • Records resident and resident related events.
  • Dining:
  • Escorts residents to and from dining room as needed.
  • Assists residents in dining room chairs.
  • Assists residents with menu.
  • Prepares plates and deliver as needed.
  • Activity Program:
  • Responsible for participation in and supporting the resident centered activity program.
  • Emergencies:
  • Responds to and acts appropriately in emergency and disaster situations.
  • Administrative Responsibilities:
  • Completes required paperwork and copies special paperwork or forms.
  • Take and record vital signs monthly; Blood Pressure, Respirations, Pulse, Temps and Weight.
  • Leadership Responsibilities:
  • Plans and completes duties with minimal direction from supervisor.
  • Works collaboratively with peers and other team members.
  • Uses tactful and diplomatic communication techniques at all times.
  • Follows up with appropriate staff, residents or other person regarding reported complaints, problems and concerns.
  • Acts as an ambassador and public relations representative to guests and other visitors.
  • Maintains all certifications required for employment.
  • Other Duties:
  • Demonstrates an appreciation of the heritage, values and wisdom of the residents and an understanding of the aging process.
  • Attends and participates in staff meetings.
  • Participates in inservices and projects as assigned.
  • Assists with quality assurance programs when indicated.
  • Assists in training new staff.
  • Qualifications:
  • Must be CERTIFIED in the State of Indiana as a C.N.A.
  • Must be able to think, act and intervene independently in both routine and emergency situations. Must be self-motivated and function as an “in charge person”
  • Must be able to relate to residents and staff in a courteous and diplomatic manner under all circumstances.
  • Must be able to work flexible hours and participate in holiday activities.
  • Working Conditions:
  • Safe, clean working environment but might be exposed to body fluids, infection, odors and behavioral problems with residents.
  • Exposed to chemicals/cleaning solutions.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Must be in good general health and demonstrate emotionally stability in order to cope with the physical, mental and emotional stress of the position.
  • Other duties as assigned.
