We're a fast-paced, expanding Community Health Center looking for a Community Based Medical Assistant to join our team and help improve the wellness of our neighbors.
***** Certified Medical Assistants will be provided a Keystone Health company Vehicle while traveling throughout the community! *****
The Community Based Medical Assistant provides patient care within the patient's home. The Medical Assistant will perform prescribed medical treatments to patients of Keystone Health during telehealth appointments with Keystone Health Providers. The Medical Assistant also works closely with our Enabling Services Department, medical providers, and primary care teams to provide short term care coordination and connection to community resources for high-risk patients. The Medical Assistant provides social support, informal counseling, and culturally sensitive education to specific targeted populations to improve patient health and general well-being.
Qualifications: Certified Medical Assistant with a minimum of two years recent experience working as a Medical Assistant. Experience working in a community related position such as health educator or a community health worker is a plus. English speaking is required. Bilingual in Spanish or Haitian Creole is preferred. A clean Driving record and a valid PA Driver’s License is a requirement for using a company vehicle throughout the community.
Normal work schedule is generally Monday through Friday. Must be willing to work some evenings and weekends occasionally to attend outreach events.
