Position Title: Community Resources Coordinator

8am - 5pm M-F, Hybrid Remote Position (some days required in office), Full-Time position

Weekends as needed based on community events. (Average 1 weekend every other month.)

The Community Resources Coordinator is responsible for coordinating volunteers who provide support and practical assistance to transitions clients and families who are coping with a life limiting illness, individuals who have requested Advance Care Planning, and a Speakers bureau to provide education and information to our community on various topics. The focus of this role is on facilitating volunteer outbound telephonic communication, providing community resources, and facilitating volunteer visits to Community Resources program participants. A primary goal of the position is to work with volunteers who offer support, follow up on client/family needs, and provide education and information about community resources as needed.

You Have:

Education – High School or GED Equivalent required; AA degree, preferred.

• 1-2 years of experience preferred in related healthcare position, preferred

Experience: Experience working with interdisciplinary team preferred. Experience working in social work or community resources setting is preferred. Excellent interpersonal, written, verbal, communications and organizational skills required. Computer skills including Word, Excel, and internet use preferred. Valid Florida driver's license and auto insurance required.
