Director of Case Management
Job description
- Reporting to the Director of Student Accessibility and Care Team Support, the Assistant Director of Student Accessibility and Student Case Manager works with complex student situations that have impacted the student’s ability to successfully function in the University community. Complex situations include, students with significant and ongoing transition issues, students with disability support needs, students needing continual oversight upon return from behavioral intervention, and students experiencing significant emotional trauma The Assistant Director will help facilitate a student’s access to, and appropriate utilization of, University and/or community services.
- The Assistant Director of Student Accessibility and Student Case Manager is a University administrative role. While the position may develop strong supportive relationships with students and staff, the specialist does not engage in individual therapeutic relationships.
- Serve as a case manager for students of concern, coordinating resources and providing on-going communication as well as conducting on-going risk assessments of students of concern
- Consult with students regarding requests for accommodations due to diagnosed mental health disability.
- Maintain concise and accurate case management records to include management of data. Reports will be generated regularly to assess for trends, inform decisions, and contribute to overall student success
- Responsible for collecting information and assessing student needs to coordinate referrals to appropriate departments at the University and/or community resources such as mental health providers and other appropriate resources
- In consultation with the Director, responsible for creating intervention plans in order to minimize and/or resolve problems and track students’ progress
- Coordinate student transitions from CNU services to external community providers if necessary (e.g. psychiatric/psychological treatment services, alcohol and drug treatment services, return to parental care)
- In consultation with the Associate Dean of Students, communicate with staff, parents, families, or designated emergency contacts regarding emergencies including hospitalizations, suicide attempts, or other concerning/high risk behaviors
- Develop and maintain a system for following up with students who have been referred to campus or community resources, providing follow-up services to ensure compliance with recommendations. Help facilitate a successful return to campus when issues are appropriately resolved
- In the absence of the Director, serve as the primary point of contact for students, parents, faculty, staff, etc regarding student accessibility and/or case management. Consult regularly with various departments on campus regarding services, access and care
- Provide information and support as appropriate to various entities on campus including the threat assessment team (TAT), behavioral intervention team (BIT), student update meetings (SUM), etc
- Be available to respond to after hours or weekend emergencies on a limited basis
- Provide proactive programming that addresses student well-being. Collaborate with departments within the Division of Student Affairs and others to assist with programs that focus on student support and intervention. Serve on committees and task forces as determined by the Associate Dean of Students
- Work collaboratively with the Division of Student Affairs to support campus wide programs multiple times throughout the academic year including admission events, move-in, Honors Convocation, Commencement and orientation. This may include participation at night and on weekends
- Develops a customer service approach to more effectively promote the “students first" initiative ensuring that all constituents are served in a fast, friendly, efficient and effective manner. Develops and maintains very positive and professional customer service and/or relations within the office/department and with all constituencies to include students, faculty, staff, guests, and employees. Demonstrates a positive and professional attitude and treats everyone with dignity and respect. Fully supports CNU Traditions and the “Students First” value at CNU and routinely goes the extra mile in providing service.
- This position is designated as a “responsible employee” who has the authority to redress sexual violence, who has the duty to report incidents of sexual violence or other student misconduct, or who a student could reasonably believe has this authority or duty.
- Serves as a Campus Security Authority. A Campus Security Authority (CSA) is defined for purposes of the reporting under the Clery Act as an “official of the institution with significant responsibility for student and campus activities”. A CSA is required to immediately report any crime that is reported to them to the University Police who will then review, evaluate, and investigate the reported crime.
- Safety issues are reviewed and communicated and goals are set to ensure a safe and healthy workplace and a reduction in work related absences. Any violation of safety rules are addressed promptly · Follow workplace safety regulations and adhere to applicable standards, processes, and programs established for your position.
- Report unsafe work conditions to your supervisor.
- Immediately report work-related incidents to your supervisor and participate in accident investigation requests.
- Performs other duties as assigned.
- Ability to assess high risk situations and apply critical thinking skills.
- Strong organizational and administrative skills.
- Ability to manage multiple tasks with competing deadlines.
- Ability to work independently with minimal supervision.
- Excellent interpersonal, written and oral communication skills as it relates to sensitive materials.
- Proficient in Microsoft Office.
- Proficient in Maxient, or other data management tools to provide best practices in case management and data management.
- Knowledgeable and skilled in developmental, psychological, and medical issues in the college student population.
