SUMMARY:
Social Services Director is responsible for planning, evaluating, developing and implementing the social services program, coordinate social services regularly with the facility staff, coordinate post-admission resident needs, participate in development of residents' comprehensive care plans, and transportation. The Director of Social Service in collaboration with other departments, medical professionals, consultants, and organizations, including government agencies and advocacy groups, develops, supports and coordinates resident care, related administrative functions, and to represent the interests of the facility to achieve the highest quality care and federal and state compliance.
RESPONSIBILITIES AND DUTIES:
- Develops Social History, Social Assessment, Care Plan to identify problems, needs, set realistic goals, problem resolution and/or needs upon Admission, compliant with State and Federal Regulations
- Participates in interdisciplinary Care Plan Meetings and in Continuous Quality Improvement (CQI) Program
- Documents Progress Notes for Resident’s Care Plan when necessary within Company Policy time frames
- Assists Residents and family members with discharge planning and discharge process
- Promotes communication assistance and support for family members and Residents; aids with TDD phones and/or other communication assistance for impaired Residents
- Attends public relations activities as requested by the Administrator and assists with community education regarding aging, Facility services and other related information
- Participates in pre-admission screening of potential Residents and with providing information to Residents and families during the admissions process
- Orients Residents to long-term care environment, Facility policies and Facility’s staff
- Interviews, orients, trains, supervises and evaluates Social Services personnel; makes work assignments, sets goals and priorities; keeps associates informed of all changes
- Manages ordering of supplies; maintains records for budget and supplies
- As requested by Administrator, attends Family Council and Resident Council Meetings; counsels with families
- Learns and complies with all Company policies, State and Federal Regulations regarding Social Services
- Must possess a valid driver’s license; assists with transporting Residents to doctor’s appointments and shopping as needed
- Ensures Residents’ personal property lists are updated at least annually
- Cooperates with and assists the Activity Department with Residents’ activities
- Completes assessments, MDS, care plans and gathers information to design activities that are multi-faceted, meet patients’ functional levels, and reflects needs and interests of each patient.
- Implement policies and procedures for the identification of medically related social and emotional needs of the resident.
- Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
- Participate in discharge planning; development and implementation of social care plans; and resident assessments.
- Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
- Review and develop a plan of correction for social service deficiencies noted during survey inspections and provide a written copy of such to the Executive Director.
- Maintain a reference library of written material, laws, standards of practice, etc., necessary for complying with current standards and regulations, and that will aid in maintaining quality social service.
- Conduct and document a social services evaluation, including identification of resident problems/needs.
- Maintain communications with caseworkers.
- Provide social work consultation to residents, families, and staff, as required.
- Participate in and/or provide in-service education sessions.
- Participate in the quality improvement process of the facility.
- Access continuing education opportunities appropriate to discipline and responsibilities
MINIMUM EXPERIENCE & QUALIFICATIONS:
- Minimum of one (1) year experience.
- Demonstrated ability to assess resident needs, develop, and implement a plan of care and coordinate the discharge planning process.
- Must possess knowledge of accreditation standard and compliance requirements.

KNOWLEDGE, ABILITIES, AND SKILLS
Knowledge of current federal and state laws and regulations that apply to the practice of nursing in a long-term care setting. Has strong organizational and time management skills and the ability to prioritize responsibilities. Ability to make presentations before top management, public groups, and staff. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to read, analyze, and interpret journals, financial reports, and legal documents. Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
SPECIAL REQUIREMENTS
Must be able to work overtime.
WORKING CONDITIONS
Works in a climate-controlled and well-lighted environment with a smoke-free policy. Works in an office and throughout the building daily. Has Category 1 risk of exposure to infectious diseases but receives training in handwashing practices and Universal Precautions and has access to personal protective equipment if direct exposure is anticipated. Is subject to interruptions, noise, and odors daily.
PHYSICAL DEMANDS

· Intermittent physical activity including walking, standing, sitting, lifting and supporting patients.
· Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment and high risk assumed
· Incumbent will be required to work at any facility and be responsible for own transportation.
· Incumbent will be scheduled based on operational need. (rotate shifts, standby, AA, on-call)
· Incumbent may be required to work extended periods of time at a video display terminal (VDT).
Incumbent may experience traumatic situations including psychiatric, dismembered and deceased patients.
