Discharge Planner
Job description
The Discharge Planner - Temporary provides assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral, mental or health care follow up after discharge from the facility. Develops continuing after care plans, based on patient needs. Responsible for setting discharge appointments for patients, including group or individual therapy. Providing all needed documentation to other providers. Acts as a liaison between referral sources, patients and counselors. Ensures all patient discharge planning documentation is clear, concise and timely.
· Associates degree or higher in nursing or human services field required.
· One or more years’ experience in a clinical setting preferred.
· One or more years’ experience in discharge planning preferred.
Job Type: Full-time
Pay: $37,000.00 - $40,000.00 per year
Schedule:
- 8 hour shift
