Aging and Long Term Support Administration seeks a Social and Health Program Consultant 4 assigned the responsibility for statewide development of partnerships and resources to increase community access for individuals with complex needs across the State's three Home and Community Services (HCS) regions. In this role, you'll focus on identifying and resolving systemic concerns and barriers documented during and after a transition to home and community-based settings. You will have significant input and responsibility for long-range program planning, policy, and goal development.
This is a primarily telework opportunity, and there may be some flexibility in terms of primary duty station assignment to other HCS offices. This job does involve state and local travel.
Some of what you'll do:
- Track, evaluate and assess the effectiveness of Roads to Community Living (RCL) participant enrollment and Nursing Facility relocation activities to identify best practices and areas of learning that will influence policy development, staff and provider training, resource development, and regional practice or relocating individuals with complex needs into community-based settings.
- Train providers and staff about existing services and best practices;
- Work closely with the Community Living and System Support (CLASS) team including the RCL Systems Change Specialist, and Housing and Resource Specialists as well as other RCL, HCS, and DDA staff. As a member of this team, you have significant input and responsibility for long-range program planning and goal development.
- Formulate and administer statewide policy to support the implementation of the above activities.
- Provide in-depth enrichment and development support to HCS, AAA and contracted providers to ensure the best possible client experience and outcomes
- Assist in achieving optimal utilization of existing long term support services
- Build and expand ALTSA contracted provider partnerships throughout the state.
Who should apply?
Applicants who possess a bachelor's degree in social work, health or social science, public administration or related field AND 5 years of experience with coordinating community services; or a master's degree and additional experience with long term care systems.
