Offer support and understanding to children and families in crisis by helping parents/caregivers gain the skills, self-confidence and access services to meet their child's complex needs.
CSO offers Training in: Trauma informed care, Motivational Interviewing, Cultural Diversity, Equity and Inclusion, Mindfulness Skills and more.
Responsibilities
- Provide weekly contact with the parent/caregiver to deliver services in accordance with an existing Outpatient or In-Home Therapy treatment plan or Individualized Care Plan (ICP) through Intensive Care Coordination.
- Deliver services in the parent/caregiver's home or community.
- Coordinate minimum weekly contact with the hub provider (Outpatient, In-Home Therapy or Intensive Care Coordination).
- Meet expected productivity level as required.
- Provide education, coaching, training and assistance to parents in navigating child serving systems (DCF, DMH, juvenile justice).
- Foster empowerment and building self-efficacy.
- Facilitate linkages to peer/parent support and self-help groups as necessary.
- Assist in identifying formal, natural support and community resources.
- For families engaging in Intensive Care Coordination services, the Family Partner will participate in Wraparound processes including completing paperwork, attending Care Plan team meetings, and communicating with Care Coordinator.
Qualifications
To be a Family Partner at CSO, you must have a high school diploma or GED as well as two years of experience working with children, adolescents or transitional aged youth. We also need our Family Partners to have experience as parent or caregiver of a youth with special needs, preferably, mental health needs. The qualified candidate must have a willingness to share their experience with child serving systems. Additionally, we ask that you possess the ability to occasionally work evenings with some flexible hours.
