OVERVIEW
The Health and Wellness Case Manager (HWCM) is responsible for direct service provision to refugees, asylees, humanitarian parolees, and other eligible populations including individuals with complex medical and mental health conditions, home visits, health orientation, health care navigation, transportation, etc. The Case Manager will collaborate and work closely with other members of the Health and Wellness (HW) team. The HWCM will handle case documentation online and in paper files as well as program reporting. This is a full-time position and reports to Program Manager of Health and Wellness.
DUTIES AND RESPONSIBILITIES
- Conduct linguistically and culturally appropriate Intake and assessment for newly arrived refugees/asylees;
- HWCM will support individuals with complex medical and mental health needs and will make an appropriate referrals and work to ensure access to appropriate care and services;
- In collaboration with HW team, HWCM will develop and deliver education sessions related to chronic diseases, sexual and reproductive health, nutrition, stress management, etc.
- Provide support, counseling, and care navigation to newly arrived families or individuals with complex medical or mental health needs;
- Work in collaboration with providers to improve the health status of refugees arriving with complex medical or mental health needs by providing added, focused case management services and care coordination;
- Ensure that refugees are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary
- Perform other duties as assigned by supervisor.
REQUIREMENTS
- Bachelor's degree with a minimum of two years relevant experience, preferably in Human Services, is preferred;
- Familiarity with social work and/or case management services and documentation is required;
- Keen attention to detail and ability to manage multiple tasks simultaneously in a fast-paced environment;
- Ability to work as a team member and independently, with a high level of self-motivation and ability to meet goals and deadlines in a fast-paced environment;
- Ability to communicate professionally via phone and email with potential clients and referral sources;
- Highly adaptable and flexible with ability to multi-task and prioritize in a demanding work environment;
- Excellent interpersonal and communication skills;
- Excellent organizational and data entry skills;
- Must have dedication to human rights of refugees, immigrants, asylum seekers, and displaced people;
- Demonstrated knowledge in the use of Microsoft Office, with proficiency in Excel;
- Must have a valid driver's license, insurance, clean driving record, and personal transportation;
- Bilingual English Arabic, Dari, Pashto, or Swahili desired.
PHYSICAL DEMANDS
- Use of manual dexterity, tactile, visual, and audio acuity.
- Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands.
- Occasional lifting (up to 25 pounds), bending pulling, and carrying.
