Fern Health is pioneering digital care for people with chronic pain - helping them avoid surgeries, return to work, and get back to doing the things they love.
A Fern Health Coach works remotely with occasional travel to educate our customers and to provide high-quality and compassionate coaching and care to individuals suffering from chronic pain. Fern approaches pain management by using a biopsychosocial approach to address the unresolved source of pain. This position will contribute to improved outcomes and increased self-management by engaging the individual in their own treatment and implementing strategies that facilitate behavioral change such as movement, exercise, sleep, and nutrition.

Responsibilities:
- Use multiple modalities, including phone, video conferencing, email, and in-person communication to provide compassionate, individualized health coaching to members with chronic pain as they work toward their health objectives and meet their goals
- Provide pain neuroscience education in order to reduce fear, stress, and anxiety related to their pain
- Educate members on best practices for improving function and pain through neuroscience education with the goal of reducing pain-related fear, stress, and anxiety
- Educate customers about Fern Health’s offerings and approach to address chronic pain.
- Encourage member engagement through outreach and follow through
- Make initial contact, conduct initial interviews, and engage in continuous communication with members throughout their treatment
- Assists members with setting SMART goals and identifying barriers to reaching their goals
- Conduct all member and customer interactions with outstanding customer service and positive communication
- Monitor adherence to care plans, progress and effectiveness and adjusts accordingly to reflect members changing needs, issues, and goals
- Create timely, complete, and concise documentation in the digital care platform, and completes all tracking and reporting requirements for evaluation and outcomes
- Escalate clinical cases based on established protocols. Escalate technical issues based on established pathways
Qualifications:
- LPN, RN, or BSN
- At least 3 years of health coaching experience
- An understanding of, or willingness to learn, the fundamentals of pain neuroscience education
- Willingness to obtain additional role certifications to meet the needs of the business as requested
- Ability and willingness to occasionally travel to meet with users and clients on site
- Excellent communication skills
Preferred Qualifications:
- Compact Licensure strongly preferred
- Certification through the National Board of Health and Wellness (NBCHWC) or willingness to achieve certification within one year
- Previous telehealth and/or digital therapeutic experience
- Bilingual language skills

$65,000 - $95,000 a year
Salary commensurate with experience, location, and qualifications.
