Hours:9:00 am- 5:00 pm, Monday-Friday

Job Responsibilities:

Responsible for pre-screening of applicants applying for HHA positions, application review, verification of all documents required by Agency's policies and procedures and processing of information utilizing HHA exchange system.

  • Coordinator is responsible for maintaining daily communication with agency personnel, clients and subcontracted agencies to efficiently manage the delivery of home care services as part of the care team.
  • Coordination of field professional, paraprofessional staff and other services provided to patients referred to the agency.
  • Schedules case coverage, always informing both patient and / or significant other of the name of time scheduled caregiver. Aims to staff cases permanently and keeps roster booked a month in advance.
  • Must keep a tangible roster of all aides and nurses availability and phone numbers for use to staff cases
  • Updates the exchange system with open cases, and replacements immediately prints opens by 11 am daily and submits to manager with strategy, priorities, and case assignment
  • Maintains and updates scheduling lists, Coordinator must keep a roster of open cases at all times and make updates to open schedules immediately.
  • Confirms all weekend cases starting Wednesday of each week
  • Coordinates scheduling of in-services for all field staffs
  • Working independently, responsible for gathering, compiling and managing data that is stored on the HHA Exchange (statistical software that they use to schedule cases and track client/staff info). Each Client Care Coordinator has their own log-in for the HHA exchange and receives training related to the HHA Exchange.Qualifications:
  • High school graduate (or equivalent)
  • One year experience in the home care agency setting
  • Proficient in MS Office
  • Must have experience with HHA exchange
  • Previous coordinating experience in home care nursing highly desired ***
