Housing Navigator - Homeless Program
Job description
Gardner Health Services is currently recruiting to fill a Housing Navigator position. This is full-time, 40 hour per week position in the Homeless Program. The Housing Navigator, as a member of a multi-disciplinary homeless assistance team will work with Catholic Charities “The Window”, GHS Homeless Project’s mobile unit and John XXIII staff , who are responsible for the whole person healthcare of homeless and at risk individuals in Santa Clara County. The Housing Navigator will work cohesively with these entities to identify and link qualified candidates with appropriate housing, via intensive case management and linkage to needed services. Linkage to GHS Downtown Resource Center and Enrollment services will round out the services with a goal of placing clients/patients in appropriate housing. The Housing Navigator operationally reports to the Homeless Project Coordinator, but will receive clinical guidance and report on case activity through the director of the Downtown Resource Center and his staff.

Minimum Qualifications:
The minimum qualifications of this position is listed below. An equivalent combination of education and experience may be considered.
- 2 years case management services
- Ability to work with diverse ethnic and multi-cultural populations
- Bi-lingual English/Spanish
- Familiar with VI-SPDAT or other homeless assessment tools preferred
- Demonstrated working knowledge of OSHA and HIPAA mandates

Compensation:
Compensation and level of position placement will be determined based on relevant experience and education.
Housing Navigator: $23.00 - $27.00/hourly
