General Description: Under the general supervision of the HIM Operations Supervisor, the Information Integrity Specialist is responsible for ensuring and improving data integrity in the electronic medical record. The information Integrity Specialist ensures all documents are entered in the correct account in a timely manner and assigned the correct form of ID. The information Integrity Specialist analyzes the entire medical record for completion, accuracy, and compliance with Joint Commission, Medical Staff and Regulations, and other regulatory standards. Additionally, performs quantitative and qualitative review of all medical record documentation by ensuring all deficiencies have been completed before archiving the chart as a permanent legal record.

Key Responsibilities:
- Retrieves medical record documentation from all areas and prepared documentation for scanning and archiving by organizing and disseminating documents that do not belong in the chart.
- Scans all medical records, verifies that the acceptable image quality is produced and ensures all documents are assigned the correct form of ID and filed into the correct account
- Verifies completion of dictated reports in the transcription software and monitors the transcription queue to ensure reports are crossing over into the EMR.
- Analyzes the entire medical record for completion, accuracy, and compliance with Joint Commission, Medical Staff Rules and Regulations, and other regulatory standards.
- Performs quantitative and qualitative review of all medical record documentation by ensuring all deficiencies have been completed before archiving the chart as a permanent legal record.
- Identifies, tracks, and analyzes trends and issues with medical record documentation and presents to appropriate parties
- Performs all duties as assigned.
Formal Education:
- Associates degree preferred. Degree/Major preferred: health information management informatics, or related healthcare technology field.
