MD Home Health is a Medicare certified Home Health agency privately owned and operated. We are celebrating 30th year in the valley, that means your dedication and hard work will be recognized.

We are seeking an experienced Home Health Intake Coordinator to join our growing team! The ideal candidate will have health care/home care experience, strong computer skills, and good customer service skills. The ideal candidate will also feel comfortable speaking with physicians, health plan case managers, and patients on the phone. At this time, MD Home Health is only interviewing candidates with strong home health experience.

Job Responsibilities:

Responsible for receiving client referrals with completed information necessary to service and bill. Must collaborate with Director of Nursing in order to validate clinical information and appropriate scheduling. Verifies insurance coverage and obtains initial visit authorization from the health plan.Ensures Intake/staffing boards are maintained with current staff availability, pending new clients & hospitalized clients.

MD Home Health is located in central Phoenix (16th Street & Glendale). The hours are Monday through Friday from 8:30 am - 5:00 pm. Hourly depends on experience in the healthcare industry.

The Ideal Candidate will have:

• Health care/home care experience
• Strong data entry/computer skills, type 40 words per min.
• Excellent Customer service skills & Experience
• Ability to interact effectively with all levels of personnel
• Effective organizational skills with attention to detail
• Professional Phone Demeanor
• Effective written and verbal communication skills in English
• Ability to work effectively in a fast paced, team environment
