Job Responsibilites:
- Demonstrate ability to engage individuals served in a welcoming, hopeful, empathetic manner regardless of disability or phase of recovery.
- Customer service.
- Responsible for answering and triaging internal and external telephone calls. Conducts initial screening of individuals served and other patrons to determine the nature of requests or problems, takes messages, refers to the appropriate parties and performs related customer service activities representing the program to the general public.
- Pulling, filing, auditing, and maintaining charts for the team assigned. This includes completing change forms and auditing and maintaining charts in accordance with Utilization Review, MCET, the Department of Community Health (DCH), OCHN and PCHP.
- Closing Team charts.
- Co-Facilitating Team meeting minutes including huddles regarding team-based care, integrated health care and other population health.
- Reviewing and sharing data reports, outcomes of team services. This includes ensuring data integrity and alignments of programs and data related to the individuals served on their team.
- Assisting with coordination of care for individuals served on the team.
- Completing vitals and documenting in E.H.R.
- Prepare and analyze metrics, provide coordination & outreach, and discuss referrals for groups & EBP’s.
- Maintaining the teams’ appointment schedule. Making/changing appointments, making changes and making follow-up phone calls to minimize “no shows.”
- Answers both general and specific inquiries regarding departmental procedures, some of which may require detailed interpretation of departmental policies and procedures.
- Operates a variety of office machines, such as personal computers, typewriters, adding machines, and calculators.
- Ensures that paperwork is maintained in accordance with Medicaid and Department of Community Health guidelines, as well as Easterseals MORC policies and procedures.
- General scanning of Team paperwork.
- Collaborates on special Team projects as assigned.
- Full compliance with mandatory training requirements.
- Demonstrates knowledge of principles of trauma informed care.
- Willingness to support and assist consumers with trauma related issues.
Minimum Qualifications:
- Be a high school graduate or have a certificate of successful completion of the General Educational Development Test.
- Have three (3) years of responsible full time clerical experience.
- Excellent customer service skills.
- Ability to make general decisions in accordance with departmental policies and procedures.
- Knowledge of modern office equipment, practices, and procedures.
- Knowledge of modern record keeping management and operations.
- Knowledge of grammar, spelling and punctuation.
- Ability to follow complex oral and written instructions.
- Computer literacy and competency with Microsoft Office Professional package including demonstrated proficiency in Access, Excel, PowerPoint, and Word.
- Must possess a knowledge and understanding of co-occurring disorders (mental health & substance abuse).
