Nursing Vault Plus is seeking a skilled
LPN to join our team and provide exceptional home care services to one of our clients located in Northeast Philadelphia. This is an exciting opportunity for a compassionate and dedicated LPN to make a positive impact in a patient's life.
Responsibilities:
Provide patient care according to the plan of care developed by the RN or

Administer medications and treatments as ordered by the physician
Monitor vital signs and report any changes to the RN or physician
Assist with activities of daily living, including bathing, grooming, and dressing
Provide trach care and vent management as needed
Maintain accurate and up-to-date patient records
Communicate effectively with patients, families, and healthcare team members
Adhere to all nursing policies and procedures
Requirements:
Active LPN license in the state of Pennsylvania
Knowledge and experience in trach care and vent management
Ability to work independently and as part of a team
Availability to work day shift Monday to Friday, with weekend shifts available
Willingness to undergo child abuse clearance and FBI fingerprinting
