LVN Case Coordinator/Case Manager
Job description
Eden HHC is looking for an experienced LVN Patient Care Coordinator within our Case Management team to join our company. The candidate must have minimum of 1 year professional home health nursing experience, care coordination and/or case management experience preferred. Our service area includes Los Angeles County, Orange County, Ventura County, San Bernardino County and Riverside County. We use Kinnser Software (WellSky). Position is Full Time for Office hours Mon-Fri 9am-5:30pm.
Candidates must have:
- Current active LVN license in the state of California
- Current CPR, Health clearance, TB clearance
- Minimum of one year of home health nursing experience, case management/care coordination experience a plus
- Demonstrates good verbal and written communication and organization skills.
- Must pass criminal background check
- Driver’s license with reliable transportation
- Proficiency in EMR using Kinnser Software (WellSky) (Willing to train a qualifying candidate)
- Good organizational skills and be able to multitask
- Positive attitude with good people skills. Supportive of other office staff
- Good computer skills
Responsibilities:
- Able to perform coordination of services in conjunction with other members of the health care team.
- Maintains accurate records.
- Reports significant findings or changes in the client/patient's condition to the Registered Nurse and patient's Physician.
- Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.
- Knowledgeable of payor source/insurance eligibility, ordering of supplies/DME's
- Reports to RN case manager and Director of Patient Care
- Be able to staff new and existing patients and coordinate patient's care needs with all disciplines involved
- Participates in case conferences
- Be able to take part in on-call rotation after hours
