Job Overview: We are seeking a dynamic and passionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team as a Health and Wellness Coordinator. In this role, you will be responsible for developing and implementing programs that promote health and wellness in our community. You will work closely with residents, families, and healthcare providers to ensure the highest quality of care and support. Responsibilities: Develop and implement health and wellness programs for residents, such as exercise classes, nutritional counseling, and health screenings. Work closely with healthcare providers to ensure that residents receive the appropriate medical care and treatments. Collaborate with community organizations to provide additional resources and support for residents. Manage and maintain accurate health records for all residents. Provide education and training to staff members on best practices for promoting health and wellness. Coordinate with other healthcare professionals to ensure continuity of care for residents. Participate in the development and implementation of policies and procedures related to health and wellness. Provide compassionate care and support to residents, including assistance with daily living activities and medication administration. Ensure compliance with all regulatory standards and guidelines related to health and wellness. Qualifications: Current LPN or LVN licensure in the state of employment. Minimum of 2 years of experience in a healthcare setting, with a focus on geriatric care. Strong communication, organizational, and interpersonal skills. Passion for promoting health and wellness in the community. Ability to work independently and as part of a team. Knowledge of regulatory standards and guidelines related to health and wellness.
