Gold Crest Care Center, located in the Pelham Gardens area of the Bronx is actively seeking a full-time MDS Assessor. Long Term Care experience required. for our Skilled Nursing Facility (Nursing and Rehab Center). Candidates must be a current NYS Registered Nurse (RN) and must have prior experience with MDS 3.0

DUTIES:

• Collects and managing assessments for the health and wellbeing of Medicare, Medicaid, and other payer residents in the facility. Ensure MDS assessments are done accurately and in timely manner that meets Federal and State Guidelines.

• Ensure CAA care plan triggers are in place

• Screen all new admission/readmission for Medicare A eligibility

· Review and update diagnosis codes and sequencing according to clinical and payer claims· Creating and distributing monthly, admission and interim MDS schedules in a timely fashion

• Participate in and oversee the timely electronic submission of all MDS

• Ensures that significant changes identified through the MDS process and are communicated to the team and MDS coordinator

• Identify residents’ acuity and specific needs to improve quality of care and optimal reimbursement for the care provided

• Works in collaboration with other IDT team and staff members to ensure proper care implementation

• Monitoring & auditing clinical records, ensuring accuracy & timeliness as assigned by MDS Coordinator

• Informing MDS Coordinator of issues related to non-compliant documentation

• Other duties as assigned by MDS Coordinator and or administrator

REQUIREMENTS:

• Must have prior experience with MDS 3.0

• Long Term Care Experience Required

• Required knowledge of PDPM and CMI

• Valid NY State RN license

• Must be in good standing with State Registry

• Must be able to work as a team member

• Must have computer skills
