Actively seeking an MDS Coordinator to join our growing healthcare team in Winter Park Care & Rehab. As the MDS Coordinator within a Skilled Nursing Facility you will be responsible for the coordination and completion of the resident assessments, and overall support of the clinical staff. MDS Coordinator Job Responsibilities:
- Identify and participate in process improvement initiatives that improve the customer experience, enhance workflow, and/or improve the work environment.
- Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
- Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
- Verify all MDS documents required either by Federal and/or State Guidelines or Organizational policy are easily available at all times is completed timely as set forth by Federal Regulations.
- Ensure Care Plans contain problem, goals, and interventions.
- Active RN License Required (may also take LPN)
- Minimum 3 Years MDS Experience in LTAC, SNF, or Hospital setting
- Must have a current/active CPR certification.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Self-motivated to learn and master the RAI Process
