Summit has a one-of-a-kind progressive continuum of care, offering services for clients in any stage of aging, from Living Well, Home Care and Therapy to Skilled Home Health, Palliative and Hospice.

To act as a bridge between these programs, we're looking to hire a Full-time Medical Records MA to start with us as soon as possible. This person would be an administrative professional who is responsible for the maintenance of patients' health documents. This position requires an understanding of how to read and identify important details in history and physical and lab reports. This role is to retrieve/review, store/catalogue and disseminate important patient documents to necessary to the team. Patient documents include their medical health records, tests/labwork, history and physical, and physician's orders. This person will be solely responsible for requesting documentation for patients from their physicians, health facilities or hospitals and following up on their transfers.

We are looking for a medical assistant preferably with an administrative background.

Responsibilities include:

  • Sending regular requests for records to existing patients' physicians including their PCP and specialists and any other provider as needed
  • Sending regular requests for records from hospitals and skilled facilities when patients transfer as well as calling each facility to follow-up on the patients condition and notify them that Summit is following
  • Obtain and file collected records in the appropriate electronic medical record system
  • Send requests to physician's offices for their signature on existing patients' orders as well as routinely follow up with the office staff when orders have not been received
  • Track and catalogue all existing orders that need to be signed versus those that have been signed
  • Routinely call existing patients to check in on their health and potential needs
  • Scan obtained H&P upon request to funnel patients to appropriate care levels
  • Working with colleagues to ensure consistency of documentation practices across the company
  • Assisting with both internal and external audits of existing documentation

Qualifications include:

  • Some higher level of education preferred
  • High school diploma required
  • Medical Assistant certificate/schooling preferred
  • Back office MA experience preferred
  • Knowledge of Microsoft Outlook, Microsoft Excel and faxing required

Job Type: Full-time

Pay: $18.00 - $23.00 per hour

Experience:

  • Back office: 1 year (Preferred)
  • Medical Assistant: 1 year (Preferred)
  • Medical office/data entry: 1 year (Preferred)

License/Certification:

  • Certified Medical Assistant (Preferred)
