The full-time Mobile Med RN utilizes professional nursing process to provide a holistic approach to individualized nursing interventions. The nurse assesses, coordinates and provides the necessary needs and duties for the individuals in service. The RN promotes mental health and physical well-being throughout treatment and crisis intervention. The RN provides individual, group, and family focused nursing interventions for individuals in service and their participating family members and supports. The RN performs nursing duties in accordance with standard agency practices and as per nursing license guidelines **$3,000 Hiring Bonus**

Minimum Qualifications:

  • Education: Registered Nurse (RN) licensed by the Commonwealth of Pennsylvania.
  • Experience: At least six months of experience working in a Psychiatric Setting preferred.

Licenses, Certifications & Clearances:

  • PA Registered Nurse (RN) license in good standing
  • Act 33/34 Clearances
  • FBI Clearance
  • Valid State of PA Class 1 Drivers License (delete if not required)
  • Access to properly insured motor vehicle for client transportation and fieldwork and a safe driving record(delete if not required)

  • Adequate technological skills to successfully utilize agency tools such as electronic medical records system, electronic prescribing system, and computer skills necessary to the job.

Essential Duties & Responsibilities

  • Assess the individual in community delivered service and provide appropriate follow up. The nurse will work with the individual to establish goals and treatment strategies. They will also collaborate with the multidisciplinary treatment team. Arranges initial psychiatric evaluation when necessary, schedules psychiatric follow up, refers to appropriate programming and advocates for services as necessary.
  • Assist, instruct and encourage medication administration and act as liaison with the prescriber.
  • Provide education and training to assist individuals in service and families with developing skills and manage their medication effectively in order to control symptoms and increase community tenure
  • Create a continuum of care progressing through the five phases of the mobile med program. Assessment/Engagement phase, Medication Administration phase, Medication Monitoring phase, Self-Medication phase, and Integration phase.
  • Provide individual nursing or supportive counseling services. Communicates with individuals in service, family members with consent, multidisciplinary treatment team, and other internal or external treatment providers.
  • Provide education and support regarding symptoms of disease and potential medication side effects.
  • Provide access to services 24 hours a day, 7 days a week.
  • Collaborates with the individual in service on Treatment Planning. Develops and maintains a Treatment Plan which reflects the individual’s needs, goals, and program phase. Reviews and updates Treatment Plan with multidisciplinary treatment as per department regulations.
  • Assesses crisis situations and responds to individual or agency crisis codes. Assist is coordinating appropriate plan of action.
  • Serves on WFS Pharmacy Utilization Review Committee (PURC) and other agency teams as interested/appointed.
  • Displays a working knowledge of agency and community resources. Refers individuals needing other services. Contacts collateral resources such as Physicians, other agencies, and supportive services. Collaborates with pharmacies, physicians, laboratories, service providers, schools, etc.
  • Maintains clinical records and completes timely documentation. Documents contacts with individuals in service and collaterals. Updates record/forms as needed and reviews ECR/charts for compliance with County, State and Program Regulations.
  • Completes all assigned trainings and stays up to date with current safety and nursing standards. Employs “universal blood and body fluid precautions” as recommended by center for disease control when administering medications or providing direct care.
  • Provides coverage for all nursing services as directed by Supervisor. Acts as a consultant for other departments in regards to medical issues within the realm of nursing education.
  • Maintains clinical competence by utilizing supervision, current literature, consultation, and staff development opportunities.
  • Adheres to Pennsylvania regulations and the Requirements set forth by Federal Agencies of OSHA and the department of transportation governing the management of infectious and chemotherapeutic waste and pharmaceutical waste.
  • Addresses all issues of fraud waste and/or abuse as they arise and proactively looks for ways to prevent instances of fraud, waste and abuse in the service delivery system and work systems of the agency as described in the agency Compliance Plan.
