Pathways Community Health Worker – Posting #25016

Hourly Rate: $20.50

Position Summary:

Full-Time position available immediately for a Pathways Community Health Worker; will travel throughout Whatcom County with a base site in Bellingham. This position requires the CHW to be able to drive to meet clients, flexibility, self-initiative, and the ability to manage time, resources, and client caseload efficiently. Under the direction of the Pathways Supervisor, this position works closely with all clients, community organizations, and other departments. This culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Through home visiting, this individual assists clients in overcoming barriers to health, social services, education and employment.

Requirements and/or Experience:

  • High School Diploma or Equivalent required.
  • Experience working with people from various backgrounds required.
  • Experience working with individuals with substance use disorders, homelessness, chronic mental illness, or people in crisis preferred.
  • Prefer one year paid or volunteer experience providing medical, social service or behavioral health case management or care coordination.
  • Bilingual English/Spanish preferred not required.
