Description:

A Physical Therapist provides and/or supervises physical therapy treatment for Neighborhood PACE participants, ages 55-100+ years. Acts as a consultant to the program for rehabilitative component and participates as a member of the Interdisciplinary team. Responsible for initial Physical therapy evaluations, reassessments, home visits and the development of a treatment plan which is consistent with participant needs. Evaluates the physical and functional status of each participant.

Essential Duties & Responsibilities:

• Provides and / or supervises the individual PT treatment of participants.
• Provides clinical supervision to the Physical Therapy Assistants and rehabilitative aides.
• Effectively documents in the EMR. Completes documentation in a timely manner.

Qualifications and Requirements:

• Requires Valid Massachusetts license certified by the American Physical Therapy Association
• Experience working with frail elderly or chronically ill persons
