CAPE is an article 31 Mental Health Clinic serving adults 21 years of age and older with a wide range of mental health needs. The Psychiatric Nurse Practitioner, will work very closely with a larger treatment team consisting of a Psychiatrist, Licensed Clinical Social Workers, Peer Advocates, and a Registered Nurse. The PNP will receive regular individual supervision and participate in case conferences involving the entire treatment team. We are looking for a PNP to work with our patients at the clinic and in our Older Adult Centers satellites throughout Queens. CAPE is a wonderful, supportive work environment providing quality therapeutic services.
PRIMARY RESPONSIBILITIES:
- Health screenings and monitoring
- Providing psychiatric evaluations and ongoing medication.
- Participating in a weekly case conference meeting with the treatment team.
- Maintain contact with and participate in supervision with the Psychiatrist.
- Maintain updated clinical records in Electronic Health Records
THE POSITION ALSO INCLUDES BUT IS NOT LIMITED TO THE FOLLOWING:
- Conduct nursing assessments for home-based patients for initial intake and continued care
- Process medication requests and prior authorization requests
- Provide patient education and blood pressure screening for groups and individuals
- Work with program psychiatrist, RN, and social workers to establish and implement a plan of care
- Obtain outside medical records from outside providers
- Conduct telephone triage (during office hours)
REQUIREMENTS: A Nurse Practitioner degree with specialized training or experience in Psychiatry. A current license to practice as a nurse practitioner in the State of New York. Ability to travel around Queens. Part-time or full-time would be considered.
