We are looking for an experienced Quality Coordinator to join our team. This position provides administrative and program support to the Performance Improvement or Risk Manager/Director. Monitors and assists quality and performance improvement efforts to prevent and effectively respond to risk events. Provide assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral, mental or health care follow up after discharge from the facility.

Responsibilities:

  • Monitor and audit program components to ensure compliance with contracts, licensing and accreditation standards.
  • Complete quality analysis reports and identify recommendation for improvement.
  • Assist in the development and oversight of program improvement initiatives and corrective action plans. Assist in the attainment and renewal of accreditation and licensing for the facility
  • Assist in the development of new programming and program expansion projects as needed.
  • Assist with program and quality related training as appropriate.
  • Discharge Surveys within 48 hours of D/C
  • Assist with discharge planning at Beach
  • Weekly AMA report, performing call backs within 72 hours of AMA at both facilities or ensuring it is done by clinical
  • Inputting level III and IV incidents into HAS for Beach program
  • Daily rounding at all SBTS houses for presence, check grievance boxes and pick up incident reports.
  • Coordination of service contractors at facilities and assisting in scheduling service calls(fridge, roof etc)
  • Alumni Coordination-Events, call backs to inquire about continued success after program etc
  • Emergency group coverage/ACA blocking

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

  • Associate’s degree within a clinical field such as nursing or counseling required. Bachelor’s degree in a clinical field preferred.
  • Three or more years’ experience working in a human services organization with implementation of performance improvement and/or evaluation activities.
  • One or more years’ experience performing work supporting Joint Commission/CARF accreditation preferred.

Job Type: Full-time

Pay: From $25.00 per hour
