Job description
POSITION SUMMARY:
Successfully promote and support the mission of Catholic Charities of Northeast Kansas through local administration of the Refugee Health Promotion (RHP) and Refugee Mental Health Initiative Programs (ReMHI). This includes supporting refugee clients with navigating medical systems, organizing health literacy and health education classes, and screening refugee clients for mental health referrals. EEO.
ESSENTIAL FUNCTIONS:
- Learns and implements guidelines for services for the Refugee Health Promotion program set forth by the Office of Refugee Resettlement (ORR) and Kansas Office for Refugees (KSOR) and stays up-to-date on relevant developments and public policies that affect refugees.
- Identify and recruit participants for health literacy and health education workshops. Complete a pre and post assessment of all clients participating in workshops.
- Maintain small caseload of clients needing assistance navigating health systems; Including primary, specialty, and mental care.
- Develop a plan of care (POC) for each client that includes clearly defined and appropriate goals to access medical assistance or insurance to address health needs.
- Utilizes RHS-15 Assessment to screen clients for appropriate health and wellness referrals.
- Assist with planning and implementation of Wellness Group activities that support mental health and wellness programming, such as community events, social adjustment activities, and outreach.
- Stays up-to-date on health-related community resources and acts as a point of contact, following up on resource needs, and trouble-shooting client barriers to accessing services when necessary.
- Tracks and reports progress towards grant outcomes and goals.
- Maintains professional conduct with clients as identified in the NASW Code of Ethics and abides by all program policies and procedures and acts as a mandated reporter.
- Other duties as assigned.
MINIMUM POSITION REQUIREMENTS:
- Must have competent oral, written, and interpersonal communication skills in English; fluency in a second language (Dari, Pashto, Swahili, or Russian) preferred.
- Bachelor’s Degree in social work or human service field or comparable experience is preferred
- Ability to work effectively in an environment that is multi-cultural and multi-lingual without preference to any certain language, ethnic, nationality, or religious groups.
- Ability to effectively utilize interpreters.
- Strong typing & internet skills as well as the ability to learn Microsoft Outlook, Excel and various database programs.
- Must be reliable, punctual, professional, and respond appropriately to people in crisis.
- Must have competent organizational skills, attention to detail and the ability to maintain confidentiality.

- Must complete VIRTUS training, background screening, drug testing, and any ongoing training as required
PHYSICAL REQUIREMENTS:
Prolonged periods of sitting at a desk and working on a computer, standing, bending and moving
Must be able to lift up to 10 pounds
Must have the physical ability to complete assignments in a variety of environments: homes of those we serve, extreme heat, humidity, dust, smoke, pet contamination, cleaning supplies, etc.
May be exposed to infectious diseases
Must have physical ability to effectively communicate with others
Must possess a valid Kansas or Missouri Driver’s license, dependable transportation, current vehicle insurance and a verified safe driving record.
