The Seminole Tribe of Florida has a Family Medicine Outpatient Clinic that offers services for the Native American communities. Our highly skilled team of physicians, nurse practitioners and nurses provide comprehensive personalized health care for patients throughout the life spectrum through preventative care, early recognition of disease states, state of the art medical management of disease and collaboration with members of health staff, other tribal departments and the tribal community.

Note the Clinic is located on the Immokalee Reservation at 1120 South 1st St., Immokalee, FL 34142. The staff schedule is full-time Monday through Friday 8am to 5pm with an hour lunch. Position Summary

The incumbent in this position is responsible for providing quality care to STOF’s patients through implementing and evaluating individual patient care plans, discharge planning and complex outpatient needs. The individual performs prospective, concurrent and retrospective evaluation of clinical information to establish medical necessity, appropriateness of services and cost effectiveness and serves as patient’s and family’s advocate during hospital stay and outpatient status. The incumbent in this position will also be required in some circumstances to complete an in-home or at hospital evaluation of patient, if deemed appropriate.

Qualifications / Requirements

Associates Degree or higher in Nursing is required. Current and valid State of Florida’s Registered Nurse License is required. Certification in Case Management is required, or has the ability to obtain within one year of employment. A minimum of two (2) years of experience working as a Medical Case Manager is required. Certification as Basic Life Support (BLS) and valid Florida Driver’s License are required. Demonstrate extensive and varied background in the health care field with knowledge of current medical standards of practice is required. Demonstrate excellent organizational and interpersonal skills. Demonstrate excellent written and verbal communication skills. Demonstrate proficiency utilizing Microsoft software packages. Ability to work a flexible schedule including evenings, weekends and holidays.
