Our client is an established and growing Hospice in Connecticut. They are looking for a Full time Registered Nurse Case Manager. This opportunity is open for immediate permanent hire.

What you NEED to know as a Registered Nurse:
- Hours: Monday to Friday, 8:30am to 5pm (May need to fill in for on call at times)
- Territories covered - Southwest CT
- Assesses the physical and psychological status of patients referred for hospice care and maintain regular nursing visits
Job Type: Full-time
Pay: $43.00 - $50.00 per hour
