Registered Nurse – Case Manager
Job description
POSITION PURPOSE
- Provide nursing services and case management to the patients of Utah Partners for Health in accordance with training, experience and licensure through screening, counseling, and education. Responsible for behavioral health integration with primary care, assist with patient registry involving mental health, substance abuse, co-morbidities, and chronic care management models according to standards of care and UPFH policies and protocols.
ESSENTIAL JOB FUNCTIONS
- Establish and provide UPFH’s case management and direct patient care for behavioral health integration with primary health care specifically for mental health illness, substance abuse, and co-morbidities.
- Responsible for sustainability of Medication Assisted Treatment program, coordinate patient care and partnerships with community.
- Responsible for maintaining The Joint Commission standards and accreditation for UPFH clinics.
- Collaborate with Health Educators to maintain Chronic Care Management and Annual Well Visit exams.
- Collaborate to ensure DSME program is sustainable and that it follows accreditation standards.
- Coordinate patient care between all Utah Partners for Health Family Clinics.
- Educate and counsel patients and their families, answer questions about follow-up, nutrition, proper care and long-term health.
- Maintain computerized patient medical records, treatment records, patient progress and proper documentation with confidentiality.
- Understand, promote and educate staff on all UPFH policies and clinical protocols.
- Lead and take part in the Behavioral Health Integration Team, and the Quality Improvement Champions Committee (QIC).
- Work with local hospitals, community clinics, social service agencies, and other programs to ensure appropriate continuity of care.
ANCILLARY JOB FUNCTIONS
- Travel to different clinic sites to support patients in those locations.
- Recognize and respond to emergencies.
- Participate in chart audits to assess success of QA/QI program. Participate in medical staff, QIC meetings.
- Coordinate the supervision and training of UPFH staff as it relates to Behavioral Health Integration, patient safety, and The Joint Commission.
- Participate in case peer review, management review, and provider education and discussions.
- Research the latest treatments to ensure the quality of patient care.
- Other duties as assigned.

- MAT coordination of care between patients, pharmacies and patient assistance programs
EDUCATION AND EXPERIENCE
- Graduate of an accredited school of nursing.
- Licensure by the State of Utah by the Board of Nursing or the Board of Medical Examiners.
- 1 year experience as a Registered Nurse in a health care center, medical office or equivalent is recommended.
- Must be certified in Basic Life Support.
- Valid driver’s license, proof of auto insurance and registration, and travel throughout clinic sites is required.
- Preferred, but not required:
- Experience using Electronic Medical Record (EMR) systems such as USIS, ECW.
- Experience working in a Federally Qualified Health Center
