Bay Nursing, Inc., a well-established Home Care and Hospice Agency located in Romeo MI, is looking for a Home Health Registered Nurse to join our Home Health Team to case manage patient care in Mt Clemens, Harrison Twp., Clinton Twp., St Clair Shores, and surrounding area.

Bay Nursing, Inc. has provided our patients a comprehensive range of home medical services including skilled nursing, occupational and physical therapy, and other home medical services for well over 25 years. Job Responsibilities

· Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es)

· Regularly re-evaluates patient nursing needs

· Initiates the plan of care and makes necessary revisions as patient status and needs change

· Uses health assessment data to determine nursing diagnosis

· Develops a care plan that establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process

· Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician

· Counsels the patient and family in meeting nursing and related needs

· Provides health care instructions to the patient as appropriate per assessment and plan

· Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient

· Prepare and maintain clinical documentation in a timely and accurate manner in compliance with company policy

· Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required

· Communicates with community health related persons to coordinate the care plan

· Participates in on-call duties as defined by the on-call policy

· Ensures that arrangements for equipment and other necessary items and services are available

· Instructs, supervises, and evaluates home health aide care provided every two (2) weeks

· Ensure individual continuing education and personal/professional development responsibilities are satisfied in a timely manner

· Maintain required licensures and certifications; provide the company with the most up-to-date copies of such documents in a timely manner

· Demonstrate working knowledge of Medicare reimbursement criteria, current documentation needs, ordered visit frequencies, dates of patient episodes, and dates recertification would be needed, if appropriate, for all patients under his/her care and supervision

· Maintain standards of practice and documentation that are consistent with Federal, State, Local, Medicare, and accreditation regulations and standards

· Demonstrate flexibility while performing job duties to ensure the highest quality of service is provided

· Utilize critical thinking skills while performing job duties; analyze work process and make suggestions for improvement

· Maintain a thorough understanding and consistently adhere to all applicable department and company policies and procedures

· Conduct job duties in a way which will reduce and avoid potential safety hazards

· Demonstrate initiative and drive to promote personal and company success

· Perform other duties as assigned by your supervisor or management team member
