Job Title: RN Clinical Coordinator - Pediatric Primary Care Clinic (PCMH)
Job Summary: The RN Clinical Coordinator at Nurture Pediatrics plays a crucial role in overseeing and coordinating clinical operations within a Patient-Centered Medical Home (PCMH) setting. This position involves providing direct patient care, managing clinical staff, ensuring adherence to PCMH standards, and promoting high-quality care for pediatric patients.
Key Responsibilities:
- Coordinate and supervise daily clinical operations within the pediatric primary care clinic, ensuring efficient and effective patient flow, and optimal utilization of resources.
- Provide direct patient care, including conducting assessments, administering medications, performing treatments, and assisting with procedures.
- Serve as a resource and mentor to nursing staff, providing guidance and support in delivering high-quality care and adhering to clinical protocols.
- Collaborate with the multidisciplinary team to develop and implement care plans for pediatric patients, ensuring coordination of care and continuity of services.
- Ensure compliance with PCMH standards, policies, and procedures, working towards continuous quality improvement and patient-centered care.
- Facilitate staff training and education related to clinical best practices, new protocols, and quality improvement initiatives.
- Assist in monitoring and managing patient outcomes, conducting quality audits, and participating in performance improvement initiatives.
- Collaborate with providers, nurses, and other healthcare professionals to promote effective communication and enhance patient care outcomes.
- Participate in the development and implementation of clinical policies, protocols, and guidelines based on evidence-based practice and regulatory requirements.
- Maintain accurate and timely documentation of patient care activities, ensuring compliance with legal and regulatory standards.
Qualifications:
- Current RN license in the state of practice.
- Bachelor of Science in Nursing (BSN) degree preferred.
- Minimum of 3 years of clinical experience in pediatric primary care.
- Strong knowledge of PCMH concepts, standards, and quality improvement initiatives.
- Demonstrated leadership skills and the ability to effectively coordinate and supervise clinical staff.
- Excellent clinical assessment, critical thinking, and problem-solving abilities.
- Proficient in electronic medical record (EMR) systems and other healthcare technologies.
- Strong communication and interpersonal skills to effectively collaborate with healthcare team members, patients, and families.
- Ability to prioritize and manage multiple responsibilities in a fast-paced clinical environment.
- Current BLS certification.
