REGISTERED NURSE--HOME HEALTH/HOSPICE 2 WEEKENDS PER MONTH

This position would provide on call support two weekend per month. Base pay plus visits worked.

ON CALL RN BENEFITS

· Medical, dental, vision, life insurance, supplemental insurance, 401K match, and PTO/Holiday Pay for full time staff.

ON CALL NURSE ESSENTIAL FUNCTIONS

· Takes call from Friday 5:00pm to Monday 8:30am two weekends per month.

· Field/triage patient/family calls and concerns

· Conducts routine visits and/or admit visits with home health or hospice patients

· Assures thorough team communication regarding patient status at shift end

· Provides community education to potential patients and their families

ON CALL RN QUALIFICATIONS

· Cheerful customer service oriented.

· Current Utah RN license (required).

· Experience in the home health and hospice settings.

· Excellent availability and response time to calls.

· Demonstrated ability to triage and apply critical thinking skills.

· Knowledge of and ability to navigate electronic medical records. HomeCare HomeBase experience a plus.

· Reliable transportation is required.
