Overview:

LifeLong Medical Care is looking for a part-time Urgent Care Registered Nurse committed to providing high-quality outpatient primary care at our Berkeley Immediate Care Center. The ideal candidate must be dedicated to building a collaborative team model of care experience working with the under-served.

As a Federally Qualified Health Center in Northern California with over 45 years of dedicated service to the community, LifeLong serves over 66,000 patients in the East Bay Area's socio-economically underserved regions. We provide medical, dental, behavioral health, and school-based services at more than 15 primary care sites across Oakland, Berkeley, and West Contra Costa County. Benefits:
We offer a competitive annual pay of $79,000-$91,500 depending on years licensed, and excellent benefits, including medical, dental, vision (including dependent and domestic partner coverage), generous leave benefits including nine paid holidays, Flexible Spending Accounts, 403(b) retirement savings plan, and commuter benefits.

Responsibilities:
  • Triages phone calls and urgent walk-ins, provides appropriate nursing care, including assessment, triage, and referral, while emphasizing patient comfort and well-being.
  • Supports satellite clinics remotely by responding to tasks from providers, triaging patient needs by phone and ensuring timely prescription refills.
  • Coordinates medical care for urgent care and satellite clinic patients through nursing best practices and by utilizing standing orders.
  • Provides well-documented, accurate and sensitive telephone and in-person drop-in advice, and well as health education and self-care instruction.
  • Coordinates with Care Transitions team to meet the needs of patients returning to the community after a hospitalization.
  • Acts as a resource for clinic nurses in properly triaging and directing patients to primary care, urgent care or the emergency room as appropriate.
  • Coordinates care closely with other team members to identify patient self-care goals and to assure that medical and psychosocial needs are met.
  • Participates in quality assurance and training of medical assistants.
  • Participates in multi-disciplinary team case management.
  • Additional duties, as assigned by Urgent Care Manager.
Qualifications:
  • Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
  • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
  • Ability to effectively present information to others, including other employees, community partners and vendors.
  • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
  • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
  • Excellent interpersonal, verbal, and written skills.
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
  • Ability to see how one’s work intersects with that of other departments of LifeLong Medical Care and that of other partner organizations.
  • Ability to delegate effectively to support staff.
  • Be creative and mature with a “can do”, proactive attitude and an ability to continuously “scan” the environment, identifying and taking advantage of opportunities for improvement.

Job Requirements
  • Bachelor’s Degree in Nursing.
  • Current and valid California Registered Nurse license and BLS.
  • Minimum one year experience in outpatient clinical practice as a registered nurse.
  • Electronic health records, Epic preferred.
  • Proficient in Microsoft office suite.

Job Preferences
  • 2 years’ nursing experience.
  • Advanced working knowledge of outpatient care, triage and clinic flow.
  • Bilingual – Spanish/English.
  • Demonstrated ability and sensitivity to provide services to persons who are disabled, homeless substance users, HIV (AIDS) infected, and/or psychologically impaired.
