We are seeking to fill the Rental Assistance Case Manager position and add valuable members to our team to accomplish our mission!

This position is part of the Regional Rapid Rehousing and Supportive Services for Veteran Families (SSVF) program, and would be responsible for coordinating rental subsidy for identified veteran households who are struggling to obtain or maintain affordable housing. The subsidy commits monthly monetary rental assistance, up to 50% of the household's rent for a period of 2 years. Veterans Inc.'s SSVF Case Manager from across New England states will identify and internally refer households to the Shallow Subsidy program.

WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:

Assessment: Conduct pre-screening of veteran referrals. Conduct evaluation assessment of all veteran clients identified as eligible. Ensuring the veterans has means to support the tenant's portion of the rent. Researching fair market rates (FMR) to determine monthly rental subsidy, maximum of up to 50% of FMR. Collect required program specific documentation.

Case Management: Providing case management to low income veteran participants throughout a 2 year period with a housing first approach. Caseload is larger than typical due to lesser intensive case management tasks. Completion of monthly check in's include: confirmation of residency or expected relocation, tenets portion of rent is in good standing, and overall confirmation of a stable household and no major housing crises. Develop, implement, and revise 90 day and 2 year housing stability plans. Assessment of recertification at the end of the 2 year period. Coordinate transition from the program recertification for continued assistance if identified.

Documentation: Completion of screening and eligibility forms, documentation of low or extremely low income, confirmation the housing unit meets SSVF standards, contributions to Housing Stability Plans, documented monthly check-ins. Forward all documentation to the Data Entry Department in a timely fashion to ensure the Homeless Management Information System (HMIS) is current. Maintain clear, concise, and up to date program documentation and files. Maintain audit ready client files as required.

Service Coordination: Working side by side internally with SSVF Rapid Rehousing and Homeless Prevention Case Managers, SSVF Healthcare Navigators and Homeless Veteran Reintegration Program Case Managers for identified barriers to maintain permanent housing or making efforts for co-enrollment. Case Managers are encouraged to build relationships with local Continuums or Care, and or VA Medical Centers to ensure the total veteran is addressed. Some households may be referred to the HUD-VASH program or another higher level of care to meet any clinical needs that were identified while in the Shallow Subsidy service

WHAT YOU MUST HAVE:

  • Bachelor's degree in Human Services, Social Work or other related field required. Military experience may be considered in lieu of a degree.
  • Minimum of two years' experience in Human Services, Vocational Rehabilitation, and Counseling, Social Work or relevant area required.
  • Experience working in a human services setting or with veteran population and/or homeless population
  • Must be able to successfully pass a CORI/Background records check.
  • Compliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.
