The Chronic Care Manager is to coordinate team-based care to provide health services to individuals through effective partnerships with patients, their caregivers and/or families, community resources, and their providers. Facilitates a “shared goal model” within and across settings to achieve coordinated high-quality care that is patient centered.

Essential Duties:
Contact point, advocate and informational resources for patients, care team, family and/or caregiver(s), payers, and community resources
Facilitate patient access to appropriate medical and specialty providers
Create, promote and monitor adherence to care plans, evaluate effectiveness, monitor timeliness of progress, and facilitates changes as needed
Cultivate and support PCP and specialty provider(s) co-management
Identify high-risk patients and ass to the registry, flagging in HER
Attend care coordinator meetings, webinars and/or meetings
Provide feedback to the clinical team regarding improvement of the care management program
Increase utilization of preventative care
Promote timely access to appropriate care
Reduce ED utilization and hospital re-admissions
Increase patients' ability for self-management and shared decision making
Increased comprehension through culturally and linguistically appropriate education
Medical reconciliation
Promote the mission, vision and values of this organization
Provides mentoring and coaching of other population health and care coordination team members
Collect data and write brief reports to meet evaluation needs of the program
Other Duties:
Health IT systems and reports
Local knowledge and connections to community healthcare and social welfare resources
Familiar with care coordination guidelines
Adhere to HIPPA regulations and maintain confidentiality
Present a positive and professional attitude
Highly organized and proficient in planning
Proficient in Microsoft Office (Word, Excel, & Outlook)
Ability to identify problems and to propose feasible solutions
Outstanding inter-personal and human relations skills
Ability to maintain accurate records

Education/Experience:
RN Licensure
Minimum of 2 years' experience preferred
