Job Description:

The Registered Nurse is responsible for providing direct nursing care to the residents. Such care must be delivered in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services designee.

Nursing Essential Job Functions

• Accurate, detailed nurse to nurse shift report
• New admissions
• Discharge patients
• Medication administration
• Daily assessment and documentation
• Change in condition care and documentation
• Communication with physicians, family and other staff members
• Requisition for labs, diagnostic testing, x-ray and other procedures
• Risk management assessment and documentation
• Written, verbal, phone physician order entry
• Follow up appointments for patients
• Arranging transportation for patient appointments
• Timely PCC charting
• IDT discharge documentation
• Keep up to date on Relias training and In-services

RN - Education & Specific Requirements

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program

• Current, unencumbered, active license to practice as an RN in this state-must report any situations that may arise in which the license could be effected
• Current BLS certification, accredited by the American Heart Association. ACLS Certification preferred
• Ability to pass a drug screening, background check and CAPS check

Qualifications

• Nursing, 1 year preferred
• PCC experience preferred
• RN current license
• BLS required

The Center at Cordera is a skilled and rehabilitation facility providing patients with physical therapy, speech therapy and occupational therapy as well as short term stay skilled nursing needs. Skilled services include wound care, IV antibiotic therapy, managing comorbidities such as diabetes congestive heart failure and respiratory failure. The Center at Cordera provides care for patients who have elective orthopedic surgery, mechanical falls, and dementia.
