Harlem Hospital Center has been a bedrock of the Harlem community since 1887. The hospital provides a wide range of medical, surgical, diagnostic, therapeutic and family support services to the residents of Central Harlem, West Harlem, Washington Heights and Inwood. Harlem is the largest hospital in the area, capable of treating the most seriously ill. Harlem provides over 90 specialized ambulatory care services, dentistry and oral surgery, behavioral health services, and community substance abuse treatment.

Under the direction of the Director of Social Work, provides social work services to patients and their families admitted to medicine. Provides direct supervision to social workers level ll and caseworkers.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

• Completes psychosocial assessments and documents all findings in the medical record per department standards.
• Collaborates with interdisciplinary team to provide safe and timely discharge planning to all patients.
• Updates medical record in accordance with department and regulatory standards.
• Provides supportive counseling patients and their families.
• Advocates for patients with community agencies.
• Flexibility with regard to work schedule.
• Meets all compliance standards,
• Trains caseworkers and level l social workers.
• Adheres to HIPAA regulations/confidentiality.
• Upholds costumer service standards when interacting with patients/families and staff.
• Participates in departmental meetings as needed.
• Adheres to infection control protocols.
• Completes reporting staff evaluations.
• Additional duties as assigned.

1. Valid license as an LCSW issued by the NYSED.
