Position Description: POSITION SUMMARY:
Under the general supervision of the Assistant Director, the Social Services Associate is part of a multidisciplinary team that provides outpatient epilepsy services in Central and South Texas. The primary role of the Social Services Associate is to assist patients and clients in accessing community resources to improve their quality of life and social functioning. Social Services Associate is responsible for providing case management, social services, and patient care services. The position entails maintaining liaison with other clinics and departments as well as other agencies. Works closely with the Clinical Services Manager and affiliated Medical Staff in the operation of the epilepsy clinic program. Some travel is required. Spanish speaking skills are required.

MAJOR JOB Duties/Responsibilities: Provides case management services to patients, clients, and their families. Performs needs assessment including Social Determinants of Health Screenings and PHQ-2 Screenings, develops services plan, and refers patients and clients to appropriate agencies.
Conducts initial meetings/phone calls with new clinic patients and clients seeking social services.
Reviews and processes new patient applications and eligibility.
Screens clients referred for emergency medications in collaboration with the Clinical Services Manager
Attends outpatient clinics (Austin, Corpus Christi, Uvalde, Harlingen, Laredo & San Antonio).
Advocates and empowers patients and clients through education, support, and guidance.
Acts as liaison between patients, providers, and community agencies.
Performs crisis intervention.
Participates in post-clinic review as part of the interdisciplinary team.
Facilitates and coordinates the San Antonio Support Group.
Coordinates the San Antonio Counseling Program.
Executes HHSC Quarterly client reporting in collaboration with Clinical Services Manager and Assistant Director.
Delivers culturally competent services.
Follows all EFCST Policies and Procedures & Department of State Health Services Policy & Procedure Manual for Epilepsy Services.
Participates in all EFCST events/activities, including evenings and weekends.
Performs other duties as assigned.

EDUCATION & KNOWLEDGE:
Minimum Education - Bachelors degree in social work or human services or equivalent experience. Must have knowledge of community resources, and specific services from the public and private sectors.

EXPERIENCE/SKILLS/ABILITIES:
Must have at least three (3) years of previous experience as a case manager/social worker. Experience working with individuals with disabilities. Good documentation skills. Ability to motivate others towards achieving goals. A clear sense of boundaries. A strong sense of and respect for confidentiality involving both patients and clients. Ability to work in a variety of settings with culturally diverse families and communities with the ability to be culturally sensitive and appropriate.

COMMUNICATION & CONTACTS:
Spanish speaking skills are required. The ability to read, and write Spanish fluently is required. Documentation and collection of subjective patient data are essential. Must have excellent organizational skills. Must be proficient in computers. Must be able to communicate effectively. Must be able to work independently. Being a team player and possessing a servant attitude is critical to the success of this position. Public speaking may be required. EFCST staff will always represent the Foundation in a professional manner.

DECISION MAKING:
Will be required to make frequent decisions with significant importance corresponding to patient/client care.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT:
Must be able to work effectively in normal and emergency situations and be able to operate patient care equipment. Requires some physical stamina to lift or move patients. Works in a modern facility with air-conditioning and heating. Will be subject to the normal hazards found in a medical clinic operation.

OTHER:
Travel to other medical clinics or facilities is required. May occasionally drive a 15-passenger van. Must have a valid drivers license, current automobile liability insurance, and a driving record with no major driving offenses. Reliable transportation is a must. Must be able to work evenings and weekends, as duties require. May occasionally participate in outdoor activities.
This is a salaried full-time position. The salary range is $40,000-$50,000.
