Job Summary
This position is a collaborative team member working on the Stable Placements Program and will provide direct support to Adult Family Home and Skilled Nursing Facility operators through training, resource connections and wrap-around service supports for program clients, with the goal of stabilizing community placements for adults discharged from hospitals after very long stays due to behavioral health challenges. The Stable Placements Program is a collaborative pilot between Catholic Charities Eastern Washington and Providence Health Care and is supported primarily through a grant from the Providence Community Benefit Fund. The expected program outcomes include, but are not limited to 1) reduced returns to the emergency department after program clients are placed in Adult Family Homes or Skilled Nursing Facilities, 2) Improved discharge timeline for program clients, 3) Improved perception of preparedness to accept program clients for Adult Family Home and Skilled Nursing Facility Operators, an 4) Improved Social Determinants of Health for program clients.
Job Duties/Responsibilities

- Works in collaboration with hospital staff and Adult Family Home or Skilled Nursing Facility Operators in providing care coordination to stabilize placements for program clients who exhibit significant behavioral health challenges.
- Reviews, updates, and facilitates implementation of behavioral health management plans for program clients.
- Maintains files for each program client, documenting services provided and case plan progress.
- Coordinates closely with Adult Family Home and Skilled Nursing Facility operators prior to and during program client placement to assure adequate preparation for accepting program client placements.
- Provides Adult Family Home and Skilled Nursing Facility operators with referrals and training connections for information related to problem-solving, conflict resolution, and interpersonal concerns as necessary.
- Conducts frequent visits on-site at facilities where program clients are placed, to assess client progress and facility operator needs.
- Provides on-call availability to assist facility operators with crisis management and intervention.
- Follows social work tenets while contributing to an interdisciplinary approach to assessment, planning of care, and intervention.
- Collects and enters client data into established entry system(s), performs entry, exit and follow up assessments as applicable.
- Performs other duties as assigned to ensure successful program implementation.
- Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
- Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.
- As a mandated reporter, follows all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.
- Adheres to the tenets of Catholic Social Teaching and Catholic Doctrine.
- Performs related functions necessary to support the mission and core values of Catholic Charities.
Job Qualifications
To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.
Education/Experience: to perform this job successfully, an individual must have a master’s degree in Social Work (MSW) or scheduled to graduate within two months. One year minimum work experience in a healthcare setting. Must demonstrate knowledge of behavioral health treatment and intervention modalities, community resources, clinical assessment and evaluation, treatment planning and the social determinants of health.
Certificates/Licenses: To perform this job successfully, an individual must have a valid Driver’s License and successfully pass background check applicable to position.
Physical Abilities: To perform this job successfully, an individual must be able to:
- Regularly sit, stand, climb, walk, talk, hear.
- Frequently lift up to 10 pounds, pull/push, carry, grasp, reach
