Service Area: McHenry County

Hours: Days – Monday through Friday

As a home health care speech language pathologist with Northwest Home Health and Rehab, your visits are tailored to your geographical location and schedule.

Speech Language Pathologist Responsibilities

• Improves or maximizes the communication of the patient.
• Provides full range Speech Language Pathology Services as ordered by physician.
• Returns the individual to optimum and productive living within the patient’s capabilities.
• Consults with physicians regarding change of treatment.
• Educate patients, caregivers and family members.
• Work closely and collaboratively with all members of the health care team.
• Inform the Clinical Manager of patient progress and clinical concerns.
• Record patient medical information in our electronic medical record system, Axxess.

Speech Language Pathologist Qualifications

• A person who has a master’s or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state of Illinois
• Two (2) years’ experience, preferred.
