Position Purpose



This position is responsible for the supervision of quality, staff development of home health nursing services for the Home Health Program. This position is responsible for working in tandem with the Director of Clinical Practice to ensure quality compliance with regulations of State, Conditions of Participation and Joint Commission as well as any other regulatory requirements for the Renown Home Health Agency. This position also includes planning, coordinating, organizing, directing and controlling professional nursing services. Responsible for developing, maintaining, improving and managing the operational systems and procedures necessary to the coordination of client care in the home health settings. In conjunction with the Director of Clinical Practice for Home Health and Hospice Care, participates in planning for present and future resources necessary for the delivery of quality cost-effective client care within the community in accordance with Federal, State, and Joint Commission requirements, and organizational goals.




Nature and Scope



This position will ensure integration and promotion of the missions and philosophies of Renown Health and the department. Effectively coordinate the day-to-day operations for the Home Health Program in respect to providing quality nursing services. It is also expected that this position will supervise assigned clinical staff; make recommendations for new hires; evaluating, discipline and recommending terminations of subordinates; assessing staffing requirements and participating in administrator on call rotation.

The Supervisor must function independently, providing the opportunity for developing standards, monitoring performance of the delivery of nursing services.

The Supervisor acts as a change agent and transformational leader, inspires a shared vision and motivates the staff to provide care within the framework of a learning organization. The Supervisor faces the major challenges of maintaining positive relationships between the physicians, the community, and the agency; for maintaining cost-effective, high quality, client care; overseeing the designing and teaching of educational programs; and working cooperatively with other departments to achieve Renown Health System and Renown Home Health goals.

The Supervisor performs, coordinates, and facilitates quality assurance and quality improvement (QI) activities with the Director of Clinical Practice for the Home Health and Hospice Program:

Provides input and recommendations for the development and implementation of the quality improvement plan for the Home Health department.

Assists in analyzes and summarizes performance improvement data from a clinical perspective and prepares and distributes reports/charts to the quality improvement committee.

Monitors Joint Commission standards compliance on an ongoing basis, identifies opportunities for improvement based on hospice quality indicators and ensures continual quality improvement with all aspects of continual readiness. Along with the Director of Clinical Practice ensures that the home health and hospice departments are in compliance with all regulatory standards (federal, state, and local).

The Supervisor may provide direct client care. He/she is accountable for the assessment; planning, implementation, and evaluation of Home Health patients.

7. Demonstrates sound nursing judgment and decision-making skills.

8. Demonstrates understanding of the principles of Home Health or Hospice philosophies and care for all Home Health or Hospice services provided.

9. Clinical knowledge of patient care practices and procedures provided by agency subordinate personnel. Knowledge of methodologies, which allows for effective client/family teaching.

10. Knowledge of federal and state regulations, and Joint Commission Standards pertaining to home health or hospice, as well as bioethical issues.

11. Working knowledge of community resources.

This position does provide direct patient care.

Education:


Must have working-level knowledge of the English language, including reading, writing and speaking English. Minimum of Associates in Nursing Degree.



Experience:


Two years nursing experience, at least one year of which in an administrative/ supervisory experience or any combination of the necessary knowledge, skills, & abilities, which would enable the candidate to meet the requirements of the position. Requires strong skills in the area of Home Health or Hospice operations, terminal care, knowledge of state and federal Hospice care rules and regulations, and management team development. Effective communication and delegation skills are required. Must accept responsibility for maintaining clinical practice skills appropriate for the direction and supervision of the Home Health services, learning and implementing the policies and procedures of the department on an on-going basis.



License(s):


Ability to obtain and maintain a State of Nevada Registered Nurse license. Valid CA or NV Driver's license and ability to pass Renown Health's Department of Motor Vehicle Report criteria. Current auto insurance.



Certification(s):


Current CPR certification by AHA standards.



Computer / Typing:


Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
